Gentle Care Hospice is currently hiring Registered Nurses -RN's and Licensed Vocational Nurses(LVN) to join our Team for San Bernardino County (Fontana, Rialto, Phelan, Moreno Valley, Yucaipa, Banning, Beaumont, etc..) The ideal applicant will be friendly, kind, caring and compassionate, and will possess excellent work ethics; initiative and strong patient care skills, and always ensure patient’s dignity and privacy. Position: Per Diem (potential part-time/full-time) Compensation: Hourly and per visit rates + mileage Duties include, but not limited to: • Follow up visits • Continuous Care • Patient and Family education • Interdisciplinary team meetings • Medication administration • Assessing the patient and family’s physical, emotional, and spiritual needs Qualifications: - Must be an LVN licensed in the state of California and in good standing - Minimum 1-2 years of hospice experience or 1 year of acute hospital experience(but not required willing to train the right person.) -Excellent verbal and written communication skills and computer proficiency - Excellent time management skills and ability to multitask and delegate duties - Reliable transportation with proof of insurance and good driving record -Professional Liability Insurance -BLS/CPR card
